The community Nurse supports the health and wellbeing of patients through the provision of high quality, culturally appropriate, holistic healthcare and counselling services. The multidisciplinary team is committed to providing an accessible and affordable primary health. The staff additionally delivers its health services in an ethical, professional and confidential manner out in the community.

A LITTLE ABOUT THE ROLE:
You will be responsible for co -ordinating with health care services to maximise the potential for effective chronic long terms conditions management to Whanau.
Provide clinical assessment and assistance to high risk / high end needs
Provide culturally competent services
Deliver appropriate health, education sessions for whanau
Your main areas of healthcare will be the Maori Mobile Nursing and Cardiac Rehabilitation
Developing personal care plans in consultation with whanau and other healthcare services
YOU WILL NEED TO :
Be a Registered Nurse with a current APC
Have experience working with Whanau in the community
Have a minimum of 2 years NZ Nursing experience
Have a full drivers license
Have a good knowledge of the West Auckland community
